Advancing health equity for underserved San Diego communities
Street Corner Care approached us to develop an app to address the growing medical needs of underserved communities in San Diego. Our team of eight, including medical consultants, developers, and designers, is focused on enhancing service delivery and fostering collaboration between volunteers and medical providers. Together, we’re working to promote health equity for underserved populations.

Why We Made This
02
Understanding
Based on Street Corner Cares' current in-person service flow, we visualize it to help all stakeholders understand the status quo, identify any flaws or opportunities, and explore how to transition the service to a hybrid model in the future.
03
Problem Statement
After reviewing the current service flow and conducting interviews and site visits with medical providers and volunteers, we have developed the following statement:
How might we provide comprehensive and equitable medical services to ensure all underserved communities in San Diego have equal access?
04
Service Blueprint
To initiate the design phase, we created a service design blueprint to visualize the hybrid service delivery flow.

Iteration
Based on user feedback, we identified a flaw in our previous design. As a result, we decided to transform the app into an on-demand medical inquiry response system.
The new flows for the three stakeholders using the app are as follows:
