Street Corner Care approached us to develop an app to address the growing medical needs of underserved communities in San Diego. Our team of eight, including medical consultants, developers, and designers, is working to improve service delivery by fostering collaboration between volunteers and medical providers while transitioning the service to a hybrid model to enhance accessibility and capacity.

Understanding
I visualized Street Corner Care's current on-site service flow to help stakeholders understand the status quo, identify flaws and opportunities, and explore strategies for transitioning to a hybrid model in the future.

After identifying flaws in the current service flow and gaining a thorough understanding of each role's needs, we developed:
